If your child has sustained an injury which restricts his or her activities while at LINX, you must provide a doctor's note clearing the child to return to full participation at LINX before he or she may do so. If your child was not seen by a doctor, a parent must provide written guidelines as to the specific activities allowed or restricted. You may use the form below to do so. By completing the form below, you are taking full responsibility for your child participating in the activities listed. However, should your child's teacher believe that allowing him or her to participate will put the child at risk of further injury, LINX reserves the right to ask the child to sit out for the activity.
